The publication of a Safeguarding Adults Review is always a moment of reflection.<\/p>\n\n\n\n

During the Board discussion, one member made a comment that will stay with me \u201cI felt I knew Martin as a person after reading the report.\u201d Of all the feedback received, that felt like the most important.<\/p>\n\n\n\n

Martin\u2019s story was complex. His final years were shaped by homelessness, alcohol dependence, the legacy of trauma and self-neglect.<\/p>\n\n\n\n

The records told part of that story. His family told the rest.
Speaking with Martin\u2019s sister reminded me that some of the most important evidence in any review sits outside the chronology. Families hold the context, the history, the strengths, the fears, the turning points and the small details that help explain how someone\u2019s life unfolded.<\/p>\n\n\n\n

Without those conversations, there is a risk that a person becomes reduced to service contacts, referrals and assessments.<\/p>\n\n\n\n

As reviewers, we have a responsibility to tell someone\u2019s final chapter with care.<\/p>\n\n\n\n

This review highlighted important learning for the partnership around:<\/p>\n\n\n\n

\ud83d\udd39 Recognising and responding to accumulating risk before concerns reach crisis point.<\/p>\n\n\n\n

\ud83d\udd39 Moving beyond narratives of \u201cdisengagement\u201d and understanding how trauma, literacy needs, executive functioning difficulties and addiction can affect a person\u2019s ability to seek help and follow through with support.<\/p>\n\n\n\n

\ud83d\udd39 Strengthening multi-agency escalation, and ensuring that the absence of a safeguarding process is not mistaken for the absence of risk.<\/p>\n\n\n\n

\ud83d\udd39 Embedding trauma-informed, persistent and relationship-based approaches to engagement.<\/p>\n\n\n\n

\ud83d\udd39 Improving the application of the Mental Capacity Act, particularly understanding the difference between decisional capacity and executive capacity.<\/p>\n\n\n\n

\ud83d\udd39 Creating stronger multi-agency oversight for adults experiencing multiple disadvantage who do not fit neatly within existing thresholds.<\/p>\n\n\n\n

My thanks go to Martin\u2019s family for their trust, and to the practitioners and Board members who engaged so openly with the review process.<\/p>\n\n\n\n

Ultimately, safeguarding reviews are about learning, improvement and prevention.<\/p>\n\n\n\n

But before they are any of those things, they are about people.<\/p>\n\n\n\n
